Shore Direct Care, LLC provides Support Coordination services for individuals who are qualified to receive Developmental Disability Waiver Programs for Adults. Through these programs, individuals with developmental disabilities and their families can access Self-Directed Services that are funded out of each State’s Department of Human Services.
